Affiliation with MCMC
Columbia Hills Family Medicine is a member of Mid-Columbia Medical Center Outpatient Clinics, composed of the offices of physicians and other healthcare providers specializing in family medicine, internal medicine, pediatrics and women’s care.
Our affiliation with MCMC allows us to best implement a patient-centered medical home model, which promotes collaboration, care coordination and integration, communication, access, quality and safety. We serve on the front lines when you need us and coordinate your care with other health care professionals and specialists when appropriate.
Our primary care services include:
Our family physicians, physician assistants, nurse practitioners and other healthcare providers are concerned with the total health care of patients. Family physicians and their teams of health professionals are trained in the diagnosis and treatment of numerous diseases and conditions, and manage both common and complex problems. They are also trained to prevent, diagnose and treat a wide variety of chronic and acute illnesses affecting patients of all ages.
Our team also includes nurses who specialize in treating and managing chronic diseases including hypertension, diabetes and COPD (chronic obstructive pulmonary disorder). We focus on prevention and education.
The family physicians of Columbia Hills Family Medicine are all certified by the American Board of Family Medicine. Board certification demonstrates a physician's exceptional expertise in a particular specialty and/or subspecialty of medical practice.
Our pediatricians specialize in the care and treatment of children from birth to 16 years of age. An important part of pediatrics is making sure children’s immunizations are up to date. Our pediatricians are certified by the American Board of Pediatrics
Our multidisciplinary behavioral health team includes a board-certified psychiatrist, psychologist and two social workers. We work closely with our clinic’s other providers to help individuals and families address mental, emotional and quality-of-life needs.
Why a relationship with a primary care provider is important
Your primary care provider plays an essential role in your personal health and in our health care system. In fact, studies show that a good relationship with a primary care provider is associated with better health for you and lower healthcare costs for our communities.
Our role as a primary care provider is to help prevent, identify and treat common problems and provide continuous care throughout your lifetime. We also coordinate other health services or specialty referrals, while serving as a point person for your care.
By having all your health information in one spot, we can best monitor your health and medications. Plus, when you’re sick, you have a consistent person you can call. Our providers always try to see their own patients, but provide coverage for you if we are unavailable.
It is important to find a primary care provider that you trust and feel you can work with for a long time. From our physicians to our front-line staff, our goal is to get you the right care when you need it.
