We Value

  • The patient as a partner in goal setting and decision making.
  • The family as an integral part of the home care team.
  • Compassionate responsiveness to the patient’s/family’s needs.
  • Our role as partner in strengthening community resources.

Mission Statement

To promote the continuum of health and well-being for the individual and family through the coordination and delivery of professional and compassionate assessments, healthcare services, and in-depth education in the home setting. To cultivate the optimal health and independence of the individual through the development of mutual healthcare goals among the patient, family, physician and home care team.

Physician Referrals

The services provided to you are determined by your medical and care needs. Before you receive Home Health care, a specific plan of care is designed for you and submitted to your physician for approval.

A Team Effort

Your home care will be directed by your physician and provided by a team of healthcare professionals that includes:

They offer instruction on care, including skin/wound management; skilled aspects of care; medical management; antibiotic administration; pain medication; IV therapy services; pediatric care; longterm chronic care, such as heart failure, COPD, and diabetes. Total joint care in collaboration with orthopaedic physicians.
  • Physical Therapists: Provide mobility assessment and treatments, build strength and mobility after injury; stroke rehabilitation; special treatments; osteoporosis and fall prevention and pediatric care. Total joint care in collaboration with orthopaedic physicians.
  • Occupational Therapists: Help with activities of daily living, self-care and safety issues; adapt homes for special needs; provide access to special equipment; teach energy saving methods for patients with lung and heart problems, including pediatric care. Total joint care in collaboration with orthopaedic physicians.

Part of a Plan

Visiting Health Services is not a substitute for treatment by your personal physician nor for personalized care that can only be provided in a hospital or nursing home. Visits are of short duration and are scheduled according to your physician’s plan of care.

Making Your Home Safe

We can evaluate your home and make recommendations for safety aids to make things easier and safer for you and those helping you with your home care. We’ll help you locate any home medical equipment you may need to buy or rent, such as a hospital bed, bathtub, grab bars or a wheelchair.

Method of Payment

Financial reimbursement for Visiting Health Services is made through:
  • Medicare / Medicaid
  • Private insurance programs
  • Veterans Administration
  • Workers Compensation insurances
  • Self pay

If the cost of a family’s home care program is to be reimbursed, Visiting Health Services will bill the insurance source directly. The patient or responsible party will receive a bill when a portion of a bill is not paid by the insurer (e.g. deductibles and co-insurance co-pay). Home care costs qualify as medical expenses and are tax-deductible, within certain limits, from federal income tax. If cost is a concern, talk to our staff. You may be eligible for funding from other sources.
